Registration Checklist

Annual registration paperwork for all new and current CA families is due by July 1, 2020. Please note that your child is not registered for the new school year until all requirements have been met.

Preschool

• Preschool Family School Covenant
• Family Financial Worksheet (each family receives their unique tuition worksheet from the school office)
• First Tuition Payment, made via cash, check, or cashiers check payable to Christian Academy
• Preschool Enrollment Contract
• FACTS Payment Agreement (Optional, though required for automatically withdrawn monthly tuition payments)
• Annual Student Registration Form, completed online through PowerSchool (once logged in, click on PowerSchool Registration on bottom-left corner of the page to locate the form)
• Emergency Care Affirmation (most be signed by both parents/guardians)
• Media Release and Consent Form (updated 2020, must be completed by all families)
• New students only:
  • Ethnicity/Christian School Experience Survey
  • Form 14 Health Form
  • DHS 908/A
  • TB Clearance Form

Elementary (K-5)

• Family School Covenant

• Field Trip T-Shirt (K-5)/PE Uniform (3-5) Order Form (Optional, though each Elementary student is entitled to one free T-shirt per school year)
• New students only:

Middle School (6-8)
